Why do you not plan on cutting down or quitting cigarettes? You do realize the long term damage you're doing to yourself?
You're asking for help but you don't seem to want to help yourself...
Nutrition, at your current weight, is the most beneficial thing you will do - execise will come secondary - if you can't walk up a flight of stairs, you're going to have a tough time "exercising off eating whatever you want"
You need a serious reality check...

FitDay - Free Weight Loss and Diet Journal -and a kitchen scale... and weigh and measure EVERYTHING you put into your mouth..
Keep a record of your daily diet and if you really do want help - then post what you're eating andyou can work on cleaning up your diet.
As for exercise -start by walking... go out for 30 minutes a day.. and gradually increase your speed in whcih you're walking
